Online Credit Request User Guide

Print Forms

Once your request is received, you will be able to print out all of the necessary forms. If you have items to return, you must include a credit request form and bill of lading with the returned merchandise. In addition, each carton being returned must have a ship- ping label attached. You may also print copies for your own records.
