1. Enter an item. You can use its: • Blish-Mize product number • Vendor product number • Customer product number • UPC number

2. Enter the quantity you want to return 3. Select the appropriate reason code 4. Provide further explanation if necessary 5. Click “Add Item” to add item to your request
